Hampshire County Council is recruiting a Reablement Service Practitioner to work across the Reablement service, including occupational therapy and the community response team. You will support people to live independently at home, using an enabling and adaptive approach to care.

As a Reablement Service Practitioner, you’ll be working across all disciplines within the Reablement service including occupational therapy and the community response team (CRT). You’ll be part of a progressive team, using an enabling, adaptive approach, supporting people to live in their own homes where possible, and in the least restrictive way.
Working Patterns
Our Service runs 07:00 - 21:00 every day of the year, therefore you’ll be required to work evenings and/or public holidays and/or weekends as part of your Normal Working Hours for which enhancements are paid and days off in lieu can be accommodated.
The current working hours for this position are a mix of day shifts 08:30 - 17:00 and evening shifts 13:00 - 21:00 on a 7-week rolling rota. Requirements to work weekends will be 1 in 7 pattern - hours 08:30 - 17:00. Working patterns are subject to change based on service requirements; any changes will be formally discussed and agreed with you.
