Job summary

Shropshire Council is seeking a Reablement Practitioner to deliver intensive short-term, person-centred care for residents aged 18 and over. The role focuses on enabling independence and wellbeing, supporting daily living activities and promoting self-care skills.

Candidates should have GCSE English and Maths (C/4 or above), a Level 3 Diploma in Adult Care with Care Certificate, and experience in domiciliary/homecare, including safe manual handling and strong written and verbal communication.

Qualifications

• GCSE English and Maths at grade C/4 or above.
• Level 3 Diploma in Adult Care and the Care Certificate.
• Experience and knowledge of domiciliary/homecare.
• Ability to undertake manual handling tasks.

Responsibilities

• Provide intensive short-term, person-centred, strengths-based care to adults 18+.
• Assist residents to regain and retain skills to live independently.
• Support with activities of daily living (self-care, cooking, washing, dressing, medication, mobility).
• Promote self-care and independence to maximise wellbeing.
