Job summary

Shropshire Council is seeking a Reablement Practitioner to deliver intensive, short-term, person-centred care across the county. You will support residents 18 and over to regain independence, promote wellbeing and maximise daily living skills.

Applicants should have GCSE C/4 in English and Maths, plus Level 3 Diploma in Adult Care and Care Certificate; experience in domiciliary care and strong written and Communication skills.

Qualifications

• GCSE C/4 or above in English and Maths.
• Level 3 Diploma in Adult Care and the Care Certificate.
• Experience in domiciliary/homecare; ability to undertake manual handling; good written, observational and communication skills.
• Funded Level 3 Apprenticeship in Adult Care including Maths and English functional skills.

Responsibilities

• Provide intensive short-term, person-centred, strengths-based care to promote independence and wellbeing.
• Assist residents 18+ to regain and retain daily living skills to live as they wish.
• Support with activities of daily living: self-care, cooking, washing, dressing, medication administration, and mobility at home.
