Job summary

West Berkshire Council is seeking a Reablement Practitioner to join the Adult Social Care Service, delivering high-quality reablement support to residents in Newbury. You will assess needs, implement plans and monitor progress to promote independence.

The role involves working with service users, carers and professionals, with DBS clearance and continuous registration with the DBS Update Service required. A flexible pattern of hours is expected.

Qualifications

• Solid understanding of reablement principles and practices.
• Excellent assessment and intervention skills.
• Strong communication and interpersonal skills.
• Ability to work autonomously and manage a caseload.

Responsibilities

• Assess individuals' needs and provide tailored support to regain independence in daily living activities.
• Implement reablement plans and monitor progress with service users, carers and other professionals.
• Conduct initial assessments and reviews to inform decision-making with timely information.
• Advise and guide individuals on overcoming barriers to independence and promoting self-care.
• Collaborate with other teams and agencies to ensure holistic care and signpost to additional services.
