Job summary

Walsall Council is seeking a Reablement Officer to support customers in their own homes, delivering a programme of active reablement to help individuals regain independence. The role is part-time (30 hours per week) and requires experience in adult social care within a multidisciplinary team.

Applicants should be prepared for DBS checks and safeguarding commitments. Salary G4: £27,274 - £29,071 pro rata to hours worked.

G4: £27,274 - £29,071 pro rata to hours worked.

If the answer is yes, then consider joining our friendly and supportive Walsall Council’s, Intermediate Care Services as a Reablement Officer.

An exciting opportunity has arisen to join Intermediate Care Service in Walsall, this is a multi-disciplinary team operated jointly between Walsall Healthcare NHS Trust and Walsall Adult Social Care.

We are particularly looking for enthusiastic and committed individuals to join our team to help shape and deliver our services, and contribute to the model of best practice. The successful candidates will be required to have experience of working in Adult Social Care and to join our Multi-Disciplinary Team.

You will be required to support customers in their own homes to develop or regain skills and confidence, to co-ordinate and deliver a programme of active Reablement to support customers to achieve their full potential, working in accordance with individualised Reablement Plans. You will work cohesively within a multidisciplinary team to maximise customer independence and reduce the need for ongoing support. Along with this, you will support customers to enjoy full access to the resources in their community, developing informal support networks.

About the role:
  • You will support/enable activities of daily living (ADL’s) including personal care, (as part of a responsive service in partnership with other agencies) as required as part of an overall support plan, ensuring that religious, cultural and gender needs are met, and personal dignity and privacy are maintained. All care interventions will be agreed with the service user and written within a support plan. The Reablement Officer will at all times support/enable activities of daily living (ADL’s) within the agreed parameters set out in the persons support plan.
  • Minimum qualification for this role is NVQ L2 Health and social care or equivalent.
