Job summary

Wigan Council’s Reablement Service is seeking a proactive Reablement Coordinator to support the delivery of an outstanding reablement service across the Wigan borough. You will coordinate referrals and work with the Wigan Together pathway home team as the reablement representative, arranging assistive technology and providing information to professionals, service users and families.

Qualifications

• You are an organised individual, proactive and passionate about supporting people to maintain their independence.
• Excellent communication and problem-solving skills, can manage competing priorities and are confident working both independently and as part of a multidisciplinary team.
• With good IT skills and attention to detail, you can maintain accurate records and respond effectively to referrals and enquiries.
• You are committed to delivering person-centred support, working collaboratively with colleagues and partners, and upholding high standards of safeguarding, confidentiality and customer service.

Responsibilities

• Coordinate and screen referrals by working with the Wigan Together pathway home team as Reablement representative.
• Arrange assistive technology services and provide information/advice to professionals, people using services and their families.
• Work closely with health and social care colleagues to ensure timely interventions that support safe hospital discharge and independence.
• Help people achieve outcomes that matter most to them.
