Job summary

Blackpool Skip Hire Ltd. is seeking a Care and Support Worker (Reablement) to help people live independently at home across Lancashire. You will conduct visits, write risk and care plans, and support medication management to ensure safe, effective care in the community.

You will also assist with discharge-to-assess pathways and carry out Trusted Assessor duties, within a strengths-based, reablement-focused approach. This is a permanent, full-time role offering 37 hours per week.

Qualifications

• This role requires experience in health and social care.
• Promoting independence and reablement is essential.
• Excellent communication and assessment skills are required.

Responsibilities

• Undertake first visits, complete risk assessments and develop person-centred care plans.
• Establish medication care plans to enable safe delivery of care in the community.
• Provide planned and unplanned support and respond to changing needs.
• Support discharge to assess pathways and timely hospital discharges.
• Contribute to reviews of care to ensure outcomes-focused services.
• Participate in Trusted Assessor (Level 3) duties, including low-level equipment fitting.
