Job summary

Wiltshire Council is recruiting a Social Care Practitioner for the Rapid Response Team in the Wiltshire area. The role delivers a rapid response within 2 hours to referrals when informal carers are unwell or hospitalised, coordinating interim packages of support to keep clients safe.

The role involves information gathering, Care Act assessments, home and placement visits, and managing a caseload while working with families, carers and health professionals to achieve positive outcomes.

Qualifications

• Relevant social care experience.
• Strong communication and ICT skills.
• Good understanding of adult social care, safeguarding and legislation.

Responsibilities

• Carry out initial information gathering and support the arrangement of interim packages of support to keep the person safe.
• Conduct home and placement visits as part of Care Act assessments and coordinating care and support.
• Manage a caseload and work with families, carers, health professionals and partner agencies.
• Focus on helping people achieve their outcomes and safeguarding vulnerable adults.
• Ensure services are delivered legally and in line with customer needs.
