Job summary

Qualifications

• Must hold a recognised Social Work qualification and current registration with Social Work England.
• Experience in adult social care or urgent response is highly desirable.
• Knowledge of strengths-based practice, risk assessment, safeguarding, capacity and consent, and person-centred care planning.
• Excellent communication, professional judgement, organisation and record-keeping skills; able to prioritise competing demands and work in a multi-disciplinary team.

Responsibilities

• Undertake strengths-based assessments and develop and review care and support plans.
• Coordinate short-term interventions and arrange appropriate packages of care, equipment, reablement, or alternative support.
• Contribute to hospital discharge and admission avoidance pathways and respond to safeguarding concerns.
