Job summary

This role focuses on prevention through strengths-based conversations at first contact to ensure the right care for the right time, helping customers stay independent.

Applicants should have a relevant Level 3 qualification and experience with vulnerable groups, plus commitment to ongoing professional development.

• Evidence of continuing professional development in a related capacity.

Responsibilities

• Support the Initial Response Team to meet increased demand.
• Carry out Rapid Response visits to those in most urgent need.
• Work with adults, their families and carers to identify social care needs and actions.
