Job summary

North Lincolnshire Council is hiring a Rapid Response Rehabilitation worker to deliver time-limited rehabilitation and reablement in people’s homes. You will support independence by following therapy plans and coordinating with health and social care professionals.

The role is 25 hours per week with weekend and bank holiday enhancements, requires flexibility and travel around North Lincolnshire, and offers strong supervision and opportunities for development within a values-based team.

Qualifications

• Experience in community-based rehabilitation or care delivery.
• Ability to follow therapy plans and work with health/social care professionals.
• Good communication and safeguarding awareness.

Responsibilities

• Provide rehabilitation support in the home and follow therapy plans.
• Respond to crisis and welfare visits to assess further support needs.
• Work flexibly including weekends/bank holidays and travel across North Lincolnshire.
