Job summary

Somerset NHS Foundation Trust is seeking a qualified Physiotherapist to join the Somerset Intermediate Care Service. You will provide unplanned, short-term interventions to prevent hospital admissions and support rapid discharge from hospital, working with local neighbourhood teams to prevent deterioration and enable safe discharge.

You will assess, plan and deliver person-centred care, manage caseloads, and support service development while promoting independence for patients in the community.

Qualifications

• Degree in Physiotherapy.
• HCPC registered.
• Car driver and full UK driving license.
• Community experience as a therapist is desirable.

Responsibilities

• Manage clinical workload and prioritise care.
• Lead and develop staff within the team; act as a positive role model.
• Deputise for the line manager in their absence.
• Contribute to service development and transformation.
