Job summary

Somerset NHS Foundation Trust is seeking a Band 6 Therapist with broad general experience to work in unplanned care services. You will provide un-planned short-term interventions to prevent hospital admission and support rapid turnaround discharge from Hospital, working as part of the new Somerset Intermediate Care Service.

You will work in a pro-active, multidisciplinary team, following relevant standards and professional behaviour.

Qualifications

• Degree in Physiotherapy.
• HCPC registered.
• Car driver and full UK driving license.

Responsibilities

• Provide unplanned short-term interventions to prevent hospital admission.
• Support rapid turnaround discharge from hospital as part of the Somerset Intermediate Care Service.
• Work with a multi-disciplinary team and operate within agreed standards and policies.
• Assess and clinically manage patients identified for RR intervention to avoid deterioration or admission.
• Speed up discharges from local A&E departments and contribute to care planning.
