Job summary

Buckinghamshire Healthcare NHS Trust is seeking a Health Care Support Worker (Band 3) to join the Rapid Response & Intermediate Care team based at Marlow Health Centre. You will provide personal and rehabilitative care to adults in their own homes and help prevent unnecessary hospital admissions while supporting timely discharge.

The role requires a friendly, can-do approach, good communication and the ability to work as part of an integrated health and social care team.

Qualifications

• Health and Social Care Level 2 Diploma or equivalent.
• Health and Social Care Level 3 Diploma or equivalent.
• Willingness to undertake Level 3 qualification in post.

Responsibilities

• Providing personal and rehabilitative care to adults in their own home.
• Being an integral member of the Rapid Response & Intermediate Care (RRIC) team, collaborating with registered Occupational and Physiotherapists to deliver rehabilitative, social and nursing care to patients.
• Taking responsibility for delivering care, equipment, supervising exercises and other delegated tasks in patients’ own homes.
• Being responsible for the completion of delegated non-clinical tasks to support the efficient running of the team.
