Job summary

Buckinghamshire Healthcare NHS Trust seeks an experienced Occupational Therapist for the RRIC Community Practitioner role (Band 6) at Marlow Community Hospital. The post is part-time (22.5 hours/week) with flexible working and service coverage from 8am-8pm, including weekends.

You will work within a multidisciplinary team to provide expert interventions to support early discharge, prevent unnecessary hospitalisation, and enable home-based recovery for patients in Buckinghamshire.

Qualifications

• HCPC registered Occupational Therapist with degree or diploma.
• Experience in community and multidisciplinary teams.
• Experience in case management and care planning.
• Flexible working in part-time post.

Responsibilities

• Provide triage, specialist assessment, review and care planning for patients and carers.
• Plan programmes of care to promote health gain and independence; actively manage cases.
• Allocate workloads within the RRIC team and coordinate with MDT professionals.
• Deliver care in a timely manner and develop clinical pathways.
