Job summary

Buckinghamshire Healthcare NHS Trust is seeking an experienced Occupational Therapist to join the Rapid Response & Intermediate Care (RRIC) team in Marlow. You will assess and manage care for patients, enabling safe discharge and home-based recovery within a multidisciplinary setting.

Registration with HCPC and a degree in Occupational Therapy are essential, with post-qualification experience in relevant settings.

Qualifications

• Essential: Registered Allied Healthcare Professional with HCPC - Occupational Therapist
• Degree/ Diploma in Occupational Therapy
• Experience of recent Continuous Professional Development.

Responsibilities

• Providing triage, comprehensive specialist assessment, review, case management and evaluation of needs of patients and carers
• Planning programmes of care to promote health gain and maximise independence, actively case managing patients through their journeys
• Allocating appropriate workloads to members of the Rapid Response & Intermediate Care (RRIC) team
• Working with a range of professionals to ensure care is appropriate and delivered in a timely manner
• Demonstrating leadership, care planning / co-ordination, and care delivery; support development of clinical pathways
