Job summary

HCRG Care Group is seeking a motivated Support Worker to join the Urgent Care Response and Intermediate Care Team in Swale. You will support patients in their own homes, aiding rehabilitation and preventing hospital admissions as part of a multidisciplinary team.

The role is full-time (37.5 hours) with 8am–8pm coverage, seven days a week, including weekend work. The base site is Sheppey Community Hospital, and you will be supported through ongoing professional development.

Qualifications

• NVQ Level 2 in Health and Social Care or equivalent qualification/experience.
• Experience in health, social care or community setting.
• Excellent communication and organisational skills.
• Compassionate and flexible approach to patient care.
• Confidence using IT systems (Outlook, Word, Excel).
• Full UK driving licence and access to a vehicle for work.

Responsibilities

• Work with multidisciplinary team to provide high-quality nursing, occupational therapy and physiotherapy support.
• Assist patients with rehabilitation programmes and daily living activities.
• Support short-term interventions for patients via Urgent Care Response and Discharge to Assess pathways.
• Undertake delegated assessments and observations within competency.
• Promote and maintain high standard of patient care and rehabilitation.
• Work independently in the community and as part of the wider team.
• Communicate effectively with patients, carers and healthcare professionals.
• Assist with administrative tasks, equipment management and general service support.
