Job summary

Buckinghamshire Healthcare NHS Trust is recruiting for therapists to join the Rapid Response & Intermediate Care team in Buckinghamshire. You will provide triage, comprehensive assessments and actively manage patient journeys to promote independence and avoid unnecessary hospital stays.

The role requires HCPC/RCN registration and experience in NHS community or hospital settings, with strong communication and leadership skills within a multidisciplinary team.

Qualifications

• Registered Healthcare Professional with HCPC or RCN registration.
• Experience of NHS community/in-patient settings, minimum 2 years post-registration.
• Experience working in a multi-disciplinary team.

Responsibilities

• Provide triage, comprehensive specialist assessment using OT skills.
• Plan programmes of care to promote health gain and maximise independence.
• Allocate workloads to Rapid Response & Intermediate Care team members.
• Collaborate with professionals to ensure care is appropriate and delivered in a timely manner.
