Job summary

Connected Health is hiring a Rapid Responder to support safe, high-quality home care across the allocated Trust zone. You will work with the Registered Manager to launch new services, assist hospital discharges and help fill care packages.

The role requires flexibility, responsiveness and a commitment to excellent care while delivering PPE, care plans and documentation to clients’ homes. As part of the Expansion Team, you will support staff, oversee safety procedures and travel between client

Qualifications

• Minimum of 6 months' homecare experience.
• Excellent verbal and written communication in English.
• Full clean driving licence. Access to a car for work.

Responsibilities

• Work with Registered Manager to support expansion of new home care services.
• Provide cover across the Trust zone as required.
• Assist with filling new care packages and hospital discharges.
• Deliver care plans, log sheets and PPE to clients' homes as needed.
• Support staff Shadowing and ensure Health and Safety procedures are followed.
• Travel between clients' homes, offices and service areas within the Trust zone.
