Job summary

Connected Health in Antrim is seeking a Rapid Responder to support safe, high-quality home care across the allocated Trust zone. You will work with the Registered Manager to launch new services, assist with hospital discharges and care package mobilisations.

This role requires flexibility, strong English communication, and a proactive, empathetic approach. A full driving licence and access to a car are essential to travel between clients' homes and service areas.

Qualifications

• Minimum of 6 months’ previous homecare experience.
• Excellent verbal and written communication skills in English.
• Full clean driving licence.
• Access to a car for work purposes.
• Ability and willingness to travel throughout the allocated Trust zone.
• Strong organisational and time management skills.
• Flexible approach to working hours and service needs.
• Ability to work closely with the Registered Manager to support new and developing services.
• Previous supervisory or senior care experience.
• Excellent interpersonal skills.
• Empathetic and professional approach to supporting clients and staff.
• Ability to work independently and as part of a team.
• Confidence in responding to changing operational priorities.
• Experience supporting service growth, new care packages or hospital discharge pathways.

Responsibilities

• Support expansion and mobilisation of new home care services.
• Provide cover and support across the allocated Trust zone.
• Support the filling of new care packages and hospital discharges.
• Undertaking emergency care cover where required.
• Deliver care plans and log sheets to clients’ homes when required.
• Deliver PPE and other essential documentation or supplies.
• Support the care team to ensure services are delivered safely, consistently and to a high standard.
• Travelling between clients’ homes, offices and service areas within the allocated Trust zone.
