Connected Health is seeking a Rapid Responder to support safe, high-quality home care across the allocated Trust zone. You will work with the Registered Manager to launch services, assist hospital discharges, and help with care packages.

The role requires flexibility, strong communication, and a commitment to excellent care, with duties including PPE delivery, documentation, and ensuring safety standards are met. A driving licence and access to a car are essential.

Formación

• Minimum of 6 months' homecare experience.
• Excellent verbal and written communication in English.
• Full clean driving licence and access to a car for work.
• Willingness to travel within the Trust zone.
• Strong organisational and time management skills.
• Flexible approach to working hours and service needs.
• Ability to work with the Registered Manager to support new services.

Responsabilidades

• Work with the Registered Manager as part of the Expansion Team.
• Support development and mobilisation of new home care services.
• Provide cover and support across the allocated Trust zone as required.
• Assist with filling new care packages and hospital discharges.
• Undertake emergency care cover where required.
• Support staff shadowing and ensure Health and Safety policies are followed.
• Check and collect daily record sheets from clients' homes.
• Deliver care plans and log sheets to clients' homes.
• Deliver PPE and other essential documentation or supplies.
• Support the care team to deliver services safely and consistently.
• Travel between clients' homes, offices and service areas as needed.

About The Role

As a Rapid Responder, you will support the delivery of safe, high-quality home care services across the allocated Trust zone. Working closely with the Registered Manager and wider team, you will assist with launching new services, supporting hospital discharges, filling care packages.

This role requires flexibility, responsiveness, and a commitment to excellent care. You will also support care staff, ensure Health and Safety procedures are followed, and assist with essential operational tasks such as delivering PPE, care plans, and documentation to help maintain smooth service delivery.

Job Requirements

• Working exclusively with the Registered Manager as part of the Expansion Team
• Supporting the development and mobilisation of new home care services

• Supporting the care team to ensure services are delivered safely, consistently and to a high standard
• Travelling between clients' homes, offices and service areas within the allocated Trust zone as required

The post holder may also be required to undertake additional duties to meet the needs and responsibilities of the company.

Essential Criteria

• Minimum of 6 months' previous homecare experience
• Excellent verbal and written communication skills in English
• Full clean driving licence
• Access to a car for work purposes
• Ability and willingness to travel throughout the allocated Trust zone
• Strong organisational and time management skills
• Flexible approach to working hours and service needs
• Ability to work closely with the Registered Manager to support new and developing services

• Previous supervisory or senior care experience
• Excellent interpersonal skills
• Empathetic and professional approach to supporting clients and staff
• Ability to work independently and as part of a team
• Confidence in responding to changing operational priorities
• Experience supporting service growth, new care packages or hospital discharge pathways
