Job summary

St Luke's Hospice (Basildon & Thurrock) invites applications for a Rapid Access to discharge (RADS) Nurse Clinician to support patients nearing end of life. You will work within the Hospice Community Services Team, providing holistic assessments and coordinating care in patients' homes or with 24-hour care providers.

You will join a multidisciplinary team delivering end-of-life care, liaising with carers, and helping patients exercise choices about their preferred place of care.

Qualifications

• Must have registered nursing qualification and be registered to practice as a nurse.
• Excellent IT, organisational and communication skills are essential.
• Driver with vehicle available for work purposes is required.

Responsibilities

• Perform holistic assessments and coordinate palliative care for patients at home or in the community.
• Collaborate with MDT to organise care and liaise with carers and families.
• Provide information, psychosocial and spiritual support to patients and carers.
• Coordinate placements with domiciliary providers and monitor ongoing care.
• Build relationships with external partners to ensure integrated care.
• Participate in rota and weekend work as required.
