Job summary

Farleigh Hospice is seeking a Head of Hospice Rapid Access Services to lead and develop a multidisciplinary team delivering rapid discharge and urgent community care for patients in their last weeks of life. You will oversee clinical leadership, on-call duty, and collaboration with partners to ensure holistic palliative care.

Qualifications

• Professional healthcare qualification (eg nurse, AHP, paramedic).
• Master's degree or equivalent, or demonstrable palliative care knowledge.
• Registered with a relevant professional body (GMC, NMC, HCPC).
• Extensive post-qualification experience, including 3 years at senior clinical level.
• Experience leading a multidisciplinary team and improving patient outcomes.

Responsibilities

• Lead the Hospice Rapid Access team, including direct line management.
• Oversee processes and performance of the team.
• Facilitate rapid discharge from hospital or provision of community care for those in last weeks of life.
• Oversee clinical assessment and regular reviews of care.
• Broker care at home, nursing homes or hospice inpatient placements.
• Provide clinical leadership for efficient palliative care services.
• Lead, develop and inspire a multidisciplinary team.
• Drive quality improvement with measurable outcomes.
• Build relationships with external partners (ICB, acute trust, Adult Social Care).
• Participate in Clinical Manager on-call rota.
• Manage flexible workforce to maintain safe, high-quality patient care.
