Qualified Social Worker, Mental Health Team

Sanctuary Personnel, an innovative and committed recruitment agency, has a new permanent position available for a Social Worker within the Adult Mental Health Team to work full time based in Harrow.

The salary for this permanent Social Worker job is up to £42,459 per annum.

Responsibilities:

1. To work in a strengths-based way with adults and their families and carers, including those who are terminally ill. This includes working with multi-disciplinary colleagues, providing advice and information, working with community supports, and liaising with other agencies, seeking the help of other professionals as necessary.
2. To have person-centred conversations with people and carers in a timely way evaluating the level of risk and an evidence-based analysis of need whilst promoting independence in accordance with agreed criteria and divisional policies.
3. To assess for and provide equipment and minor adaptations to promote independence, and to monitor these as necessary, seeking advice where appropriate.
4. To maintain and take responsibility for effectively managing a personal caseload including complex cases in consultation with the Team Manager.
5. To work as a member of a multi-disciplinary team liaising with other professionals including health and the voluntary sector ensuring that there is the fullest cooperation with other team members, to provide an easily accessible service geared to meeting people's needs.
6. To ensure, along with other colleagues in other parts of the department, that people and their carers are involved in all decisions and advocates are recommended where appropriate.
7. To assist in the planning of services in consultation with other professionals, to enable people to remain as independent as possible.
8. To prepare and present reports in a clear, concise, and logical way to a high standard, which contribute to multi-agency and multi-disciplinary conferences and meetings and court hearings and attend these forums.
9. To seek advice and guidance on matters that need resolution and report shortfalls in services to line managers.
10. To offer advice to other workers in the multi-disciplinary team where a social worker has a particular level of skill or knowledge.
11. To undertake any other duties that may be required commensurate with the general level of responsibility of the post.

Requirements:

1. Social work qualification with registration with Social Work England.
2. Experience in managing social work caseloads and supervising others.
3. Experience of community and/or group work.
4. Knowledge of children's health and social care systems, agencies, and relevant legislation.
