The 3B Hospital Social Work Team provides assessment and discharge support for residents across Westminster, Royal Borough of Kensington and Chelsea, and London Borough of Hammersmith and Fulham. The team at Charing Cross collaborates with Imperial Trust and covers Charing Cross and Hammersmith Hospitals, including a Neuro Rehabilitation ward at Charing Cross.

You will join a collaborative management team overseeing the 3B Hospital Team, including Chelsea and Westminster Hospital in RBKC and St Mary’s Hospital in Paddington, as well as the Step Down Team in LBHF.

Your responsibilities include completing Care Act assessments, facilitating safe discharges, and working in partnership with NHS and health colleagues across various trusts and community teams. You will manage a caseload, support senior practitioners and the Team Manager, supervise student social workers, and ensure high standards of social work practice compliant with the Care Act 2014 and the Mental Capacity Act 2005.

This role requires close collaboration with NHS and health partners in acute trusts and community health services, including CCGs, Continuing Health Care Teams, CLCH, and CNWL Home First Teams.
