  • Assess, plan, implement and evaluate interventions and care packages for a caseload.
  • Work in partnership with children, young people and families to meet identified needs.
  • Collaborate closely with other members of the CYP Public Health Service 0-19.
  • Maintain strong working relationships with partners and community organizations.
  • Autonomously evaluate care, take appropriate actions and drive improvements.
  • Support and train junior team members, including students.
Job description
Overview

We are looking for an enthusiastic and motivated Nurse to join our Poole West 0‑19 Locality team, covering Canford Heath, Old Town, Hamworthy and Turlin Moor, as a Public Health Staff Nurse 0‑19. This role is part‑time (22.5 hours per week) with core hours of 09:00‑17:00 and offers a hybrid working model, balancing home‑based and locality coverage.

Key Responsibilities
  • Participate in the delivery of the Healthy Child Programme across family hubs, clinics, family homes, schools and early years settings.
  • Assess, plan, implement and evaluate interventions and care packages for an allocated caseload, promoting independence and autonomy.
  • Work in partnership with children, young people and families, and within a multi‑disciplinary team, to meet identified needs.
  • Collaborate closely with other members of the CYP Public Health Service 0‑19 to provide safeguarding and support services, using evidence‑based, early intervention strategies.
  • Maintain strong working relationships with partners, colleagues and community organisations, providing health advice and management support to clients/service users.
  • Autonomously evaluate care, take appropriate actions and drive improvements in quality standards.
  • Support and train junior team members, including students, under the guidance of the specialist community public health nurse.
Qualifications & Requirements
  • Registered Nurse with a relevant NHS nurse qualification.
  • Licensed to practice in the UK; candidates requiring current Skilled Worker sponsorship are ineligible to apply.
  • Enhanced Disclosure and Barring Service (DBS) check – the Trust will cover the cost. Those with a DBS update service may present a valid certificate instead of a new check.
Working Conditions
  • Part‑time post of 22.5 hours per week with flexible core hours (09:00‑17:00).
  • Hybrid working model – remote home working combined with coverage of the Poole West locality.
