Job summary

Codi Group is seeking a compassionate support worker to provide person-centred support to adults with complex needs, trauma, and varied backgrounds. Working within a Psychologically Informed Environment (PIE) framework, you will help create tailored support plans and assist individuals to move toward their goals.

You’ll engage daily with service users, document progress, run tenancy and wellbeing workshops, coordinate with external agencies, attend appointments, and perform welfare checks.

Qualifications

• Experience of working with people with complex backgrounds (mental health, substance misuse, offending history)
• Ability to self-manage a caseload
• Desire to help people live as independently as possible
• Strong initiative and ability to work alone as well as in a team
• Ability to handle conflicting and difficult situations

About The Role

Working in a team, you will provide support to a diverse range of people (18+). All are from various backgrounds, currently experiencing trauma and having complex needs. These can range from alcohol or substance misuse, mental health, anti-social behaviour. Through using a Psychologically Informed Environment (PIE) framework & working with external agencies you will create person-cantered support plans that will support the individuals to achieve their goals and aspirations.

Your Day Will Consist Of

• Engaging and interacting with the people we support
• Writing and following a support plan with the individual you are supporting
• Delivering workshops relating to tenancy management and health and wellbeing
• Liaising with external agencies to arrange multi-disciplinary meetings
• Supporting with attending appointments with the people we support
• Welfare checking
• Addressing any additional needs, the people we support might have

Skills And Experience Needed

• Experience of working with people that have complex backgrounds (mental health, substance misuse, offending history etc)
• Ability to self-manage a caseload
• A passion to work with and enable people to live as independent a life as possible
• A strong initiative and ability to work alone as well as being a great team player
