Key Responsibilities:

Develop and implement individualised support plans.
Provide one-on-one support, guidance, and advocacy.
Work collaboratively with local healthcare providers, and housing services.
Conduct regular assessments and reviews to monitor resident's progress.
Maintain accurate records and documentation in line with organizational policies.

What we need from you:

Experience in a similar role or working with vulnerable groups.
Knowledge of issues related to homelessness and substance misuse.
Strong communication and interpersonal skills.
Ability to work independently and as part of a team.
Empathy, resilience, and a non-judgmental approach.
Relevant qualifications in social work, psychology, or related fields are desirable.
