Job summary

The Forward Trust is recruiting a Health and Wellbeing Practitioner to support adult men and young men in the Criminal Justice System. You will manage caseloads, deliver 1:1 and group sessions, and work with partner agencies to provide personalised wellbeing support aimed at positive change and successful resettlement.

• Experience in delivering personal wellbeing services.
• Ability to assess individual needs and develop care plans.
• Flexible and able to travel between projects.

Responsibilities

• Manage an allocated caseload and provide consistent support.
• Build positive relationships with service users.
• Assess needs and develop care and support plans.
• Deliver 1:1 sessions and group-work programmes.
• Support clients to develop coping strategies and wellbeing.
• Coordinate with partner agencies to ensure continuity of care.
• Prepare individuals for release and resettlement.
• Record outcomes and monitor impact.
• Travel to other projects as needed.
• Work flexible hours including evenings/weekends with notice.
