Job summary

Forward Trust is seeking a dedicated Health and Wellbeing Practitioner to support adult men and young men within the Criminal Justice System in Cambridgeshire.

You will manage caseloads, build trusted relationships, assess needs, and deliver 1:1 and group wellbeing sessions, collaborating with partner agencies and ensuring continuity of care.

The role may involve travel between projects and occasional evenings or weekends to meet service needs.

Qualifications

• Experience working with adults in the Criminal Justice System.
• Ability to assess needs and develop care plans.
• Strong communication and collaboration with partners.

Responsibilities

• Manage an allocated caseload and ensure consistent support.
• Build professional relationships with service users.
