Job summary

Forward Trust is seeking a dedicated Health and Wellbeing Practitioner to support adult men and young men within the Criminal Justice System in Cambridgeshire.

You will manage caseloads, build trusted relationships, assess needs, and deliver 1:1 and group wellbeing sessions, collaborating with partner agencies and ensuring continuity of care.

The role may involve travel between projects and occasional evenings or weekends to meet service needs.

Supporting People on Probation Navigator

Location: Cambridgeshire

Salary: £26,000 per annum

Vacancy Type: Permanent

About The Role

We are looking for a dedicated and client-focused Health and Wellbeing Practitioner to join our team, supporting adult men and young men within the Criminal Justice System.

Working alongside a team of practitioners, subcontractors and partner agencies, you will provide personalised Personal Well Being Services designed to support individuals to make positive changes and improve their wellbeing.

The support you provide will be tailored to each individual's needs and may cover:

  • Family and Significant Others
  • Lifestyle and Associates
  • Emotional Wellbeing
  • Social Inclusion
What You'll Be Doing

As a Health and Wellbeing Practitioner, you will:

  • Manage an allocated caseload, ensuring individuals receive appropriate and consistent support.
  • Build positive, professional and trusting relationships with service users.
  • Assess individual needs and develop appropriate care and support plans.
  • Provide advice, guidance and practical support based on individual circumstances.
  • Deliver structured 1:1 sessions and group-work programmes across the Personal Well Being Services.
  • Support individuals to develop positive coping strategies, improve their emotional wellbeing and make sustainable lifestyle changes.
  • Work collaboratively with colleagues, subcontractors and partner agencies to provide joined-up support.
  • Provide transitional support for individuals moving from prison into the community, helping to prepare them for release and resettlement.
  • Work closely with Health and Wellbeing agencies both within the prison and in the community to ensure continuity of care.
  • Contribute towards service targets and outcomes set by Forward and local commissioners.
  • Accurately record and monitor outcomes, demonstrating the impact of the support provided.
  • Develop and maintain effective working relationships with partner organisations and other professionals.
  • Adapt your approach to meet the changing needs of individuals and the service.
  • Travel to other projects within your cluster when required to support continuity of care.
  • Work flexibly, including occasional evening and weekend shifts where required and with reasonable notice.

All probation-based roles are subject to enhanced DBS checks and HMPPS security vetting. These checks can take up to 6- 16 weeks to complete. Any offer of employment will be conditional upon the successful completion of both checks.

Please note if you have lived overseas within the last 5 years then checks may take longer.

  • Comfortable managing a caseload and working independently while also contributing effectively as part of a wider team.
  • Organised and able to manage competing priorities effectively.
  • Committed to partnership working and delivering positive outcomes.
  • Flexible and adaptable, with a willingness to travel between projects when required.
  • Able to work sensitively and professionally with individuals involved in the Criminal Justice System.
  • Committed to promoting inclusion, wellbeing and positive change.
