Job summary

The Forward Trust in Hertfordshire is seeking a Health & Wellbeing Practitioner to support people in prison by providing client-centred care for substance misuse and mental health needs. You will manage a caseload, undertake comprehensive assessments and develop SMART care plans tailored to each client.

You will deliver 1:1 and group sessions, use motivational interviewing, and work with a range of partners to ensure smooth transitions back into the community.

Qualifications

• Significant experience in substance misuse and/or mental health services.
• Ability to build trusting, professional relationships with clients.
• Experience delivering 1:1 and group psychosocial interventions.
• Ability to identify and manage risk and develop SMART care plans.
• Strong skills in motivational interviewing to support behaviour change.

Responsibilities

• Provide harm reduction advice and interventions.
• Complete comprehensive assessments, risk assessments and SMART care plans.
• Deliver structured 1:1 and group sessions on substance misuse and mental health.
• Use psychosocial techniques to support positive behaviour change.
• Assist clients to set recovery goals and build resilience for life beyond prison.
• Provide effective throughcare and community re-entry support.
• Collaborate with internal and external services to ensure continuity of care.
• Maintain accurate case notes and reports; contribute to service targets.
• Produce high-quality progress reports for clients when required.
