Job summary

Central and Northwest London Mental Health NHS Trust is recruiting Band 6 Assertive Outreach Practitioners to deliver intensive, home‑based outreach for individuals with severe mental illness at risk of disengagement. You will work in the Assertive Outreach Team (AOT) delivering proactive, patient‑facing care in community settings.

The role requires risk assessment, care planning and coordination across CMHTs and crisis services.

Qualifications

• Registered mental health professional with current registration.
• Experience with complex mental health needs and high-risk service users.
• Experience in risk assessment, formulation and safety planning.
• Experience of community-based working, including home visits.

Responsibilities

• Deliver intensive outreach, including regular home visits and community-based engagement.
• Engage hard-to-reach individuals using persistent and flexible approaches.
• Undertake clinical assessments, risk management and care planning.
• Act as care coordinator ensuring continuity of care.
