Job summary

Leeds and York Partnership NHS Foundation Trust in Leeds is seeking a Band 5 Occupational Therapist to work in the community within the Assertive Outreach Team, delivering therapy to people with complex mental health needs.

You will collaborate with a multidisciplinary team, run group programmes, assess and plan care, and promote social inclusion while maintaining risk awareness and safe practice.

Qualifications

• Qualified Occupational Therapist with HCPC registration.
• Experience of working with service users with complex psychosis.
• Ability to work in the community and engage diverse service users.

Responsibilities

• Deliver occupational therapy interventions within the Assertive Outreach Team in the community.
• Promote and facilitate a group programme.
• Engage with other community teams and services to support care pathways.
• Ensure safe, patient-centred care and collaborative planning with MDT.
