Job summary

NHS Foundation Trust in Calderdale/Kirklees seeks Band 6 mental health practitioners (RMN, OT, Social workers) for its CAMHS Crisis and Intensive Home-Based Treatment Team. You will deliver emergency assessment and intensive community-based interventions to children and young people with mental health needs.

The role involves coordinating care, risk assessment and collaboration with partners, under supervision, with flexible working options and NHS benefits.

Qualifications

• Experience in CAMHS or mental health crisis care.
• Ability to coordinate care for complex cases and develop care plans.
• Experience with risk assessment and safeguarding principles.

Responsibilities

• Provide emergency assessment and intensive treatment and outreach to children and young people.
• Deliver timely access to specialist CAMHS services for crisis cases.
• Coordinate care planning, risk assessment and liaison with other professionals.
• Maintain accurate records and participate in supervision and governance.
