Job summary

Blackstone Recruitment Limited seeks a Band 6 Community Mental Health Practitioner to deliver recovery-focused, person-centred care within an Adult Community Mental Health Team. You will assess, plan, deliver and review care across the community pathway, working with service users, carers and a range of health and social care partners.

You will assess, formulate and implement care plans, using evidence-based interventions, risk management and crisis pathways as part of a multidisciplinary team.

Qualifications

• Registered with a relevant professional body to practice.
• Experience delivering biopsychosocial assessments with complex mental health needs.
• Ability to develop person-centred care plans in collaboration with service users and carers.

Responsibilities

• Undertake comprehensive biopsychosocial assessments of adults with complex mental health needs.
• Develop person-centred formulations and care plans with service users and carers.
• Deliver evidence-based mental health interventions and risk management.
• Coordinate care across MDTs and partners to ensure continuity of care.
• Attend and contribute to clinical huddles, supervision and reviews.
