Job summary

University Hospitals Coventry and Warwickshire NHS Trust is seeking a Proactive Care Lead to coordinate high intensity service users and develop personalised care plans to improve outcomes and reduce unnecessary demand. You will work across primary care, community, acute and voluntary sectors, leading multidisciplinary teams to deliver person-centered care.

The role requires strong leadership, excellent communication, and a commitment to improving patient experience.

Qualifications

• Registered healthcare professional (RGN, HCPC or equivalent registration).
• Independent prescribing and Advanced Health assessment qualification or plan to complete within 12 months.
• Degree-level qualification or equivalent experience.
• Evidence of continuing professional development.
• Leadership and management qualification or equivalent experience.
• Master's level study or working towards Master's level qualification.
• Training in quality improvement, service redesign, population health management or personalised care.

Responsibilities

• Identify and support high intensity service users with holistic assessment and coordinated care planning.
• Undertake comprehensive assessments to understand drivers of high service utilisation.
• Develop and review personalised care plans to promote self-management and reduce avoidable attendances.
• Collaborate with multidisciplinary teams to coordinate interventions and ensure seamless care.
• Act as a key contact for high intensity users, providing case management and navigation.
• Identify patients for care in alternative settings to ensure appropriatecare.
• Monitor service utilisation patterns and identify risks and opportunities for intervention.
• Support strategies and pathways to reduce health inequalities and enhance integrated care.
• Maintain records, collect data, and contribute to service evaluation.
• Build relationships with patients, carers, families and partners to promote person-centred care.
