• Proactively identify high intensity service users across acute, community, primary care and urgent/emergency care.
• Undertake holistic assessments to understand drivers of high service use and plan alternatives.
• Develop and regularly review personalised care plans to promote self-management and reduce attendances.
• Collaborate with multidisciplinary teams across sectors to ensure seamless care delivery.
