Job summary

University Hospitals Coventry and Warwickshire NHS Trust is seeking a Proactive Care Lead to drive integrated neighbourhood care, coordinating care across primary, community, acute and voluntary sectors. You will champion person-centred care, support population health management and lead improvements to reduce avoidable hospital admissions.

The role requires clinical and operational leadership to deliver proactive, preventative care, develop neighbourhood services, and work with system partners

Qualifications

• Requires clinical leadership experience within multisector health and social care
• Experience coordinating care for people with complex health and social care needs
• Ability to lead service development and population health management approaches
• Strong communication and collaboration with multidisciplinary teams

Responsibilities

• Lead strategic development, implementation and evaluation of proactive care services across Integrated Care and community settings.
• Develop proactive, preventative and anticipatory models of care for complex needs.
• Contribute to service transformation to improve integration among health, social care, mental health, acute and voluntary partners.
• Develop policies, pathways, standards and SOPs for proactive care and personalised care planning.
• Act as a subject matter expert to advise senior leaders and system partners.
• Lead development of innovative care models to improve outcomes and reduce admissions.
