Job summary

Shore Medical is seeking an enthusiastic and compassionate Integrated Care Team GP to support our housebound and care-home population. The post holder will provide proactive, coordinated care to patients with complex health needs, frailty and long-term conditions, working alongside the wider health and social care team.

The role focuses on improving patient outcomes and reducing avoidable hospital admissions.

Qualifications

• Worked within a UK GP Practice.
• Working in Primary Care in an area with significant health problems.
• EPS prescribing.

Responsibilities

• Provide comprehensive, person-centred GP care to housebound and care-home patients.
• Hold a weighted list of patients per session with prioritisation according to need and complexity.
• Undertake proactive reviews of patients with frailty and multimorbidity.
• Provide face-to-face and remote clinical consultations as appropriate.
• Assess, diagnose and manage acute and chronic medical conditions.
