Job summary

Yorkshire Health Network (YHN) seeks an experienced General Practitioner to lead the Proactive Frailty Team, working 12 hours per week to deliver comprehensive medical care to referred patients.

The Lead GP will oversee the multidisciplinary team during operating hours and provide clinical guidance to colleagues, while coordinating with Operational Managers and the Clinical Lead on service delivery.

Qualifications

• GP with leadership of a multidisciplinary team.
• Experience in frailty and complex care for older adults.
• Part-time commitment of 12 hours per week.

Responsibilities

Job description

Yorkshire Health Network (YHN) is seeking an experienced General Practitioner (GP) to work 12 hours per week, leading the Proactive Frailty Team (PAFS) and delivering comprehensive, high-quality medical care to patients referred to the service., The Lead General Practitioner: Proactive Frailty Team is responsible for medically leading the multi-disciplinary team during the hours that the service is operational. They will be required to provide support and advice to other clinical and non-clinical colleagues as required and to work with the Operational Managers and Clinical Lead on the delivery of the service, escalating issues as required.

Yorkshire Health Network (YHN) is a federation of 16 GP practices within the Harrogate and Rural area of the Humber and North Yorkshire ICB (integrated Care Board).
