Job summary

Hedena Health Ltd is seeking a Paramedic or Advanced Practitioner to join the Proactive Care Team (PACT) in Oxford. The role involves direct patient care, including telephone and in-person assessments, with a significant element of home visiting to support complex patients in the community.

The role emphasizes multidisciplinary collaboration with GPs, nurses, pharmacists and social care to prevent hospital admissions and improve outcomes.

Qualifications

• HCPC registered Paramedic or NMC registered ANP.
• Experience managing a diverse and changing caseload.
• Proactive care commitment and patient-focused.
• Experience with Windows and MS Office applications.
• Must have current UK DBS clearance or clear background.

Responsibilities

• Provide direct patient care via telephone assessments, face-to-face assessments and home visits.
• Collaborate with MDTs across primary and secondary care.
• Support proactive care to prevent hospital admissions for complex patients.
• Work within the PACT at Bury Knowle Health Centre, Headington.
