Job summary

Oxleas NHS Foundation Trust is seeking a Band 7 Prison Services Clinical Pharmacist to lead medicines optimisation within HMP Guys Marsh and our integrated healthcare team. You will deliver structured reviews, manage repeat prescribing and provide medicines advice in a secure environment.

This role requires strong clinical knowledge, leadership, and collaboration with GPs, nurses, and multidisciplinary teams. An enhanced DBS check and prison vetting are required; sponsorship is not offered.

Qualifications

Master’s degree in Pharmacy or equivalent and GPhC registration.
Diploma in Clinical Pharmacy or equivalent, or currently working towards.
At least three years’ experience delivering clinical pharmacy services in relevant settings.

Responsibilities

Coordinate structured medication reviews and minor ailment clinics.
Manage repeat prescribing and reauthorisation through SystmOne.
Provide medicines advice, adherence support and clinical reviews.
Support patients with long-term conditions and high-risk medicines.
Make evidence-based recommendations to prescribers.
Support medicines governance, formulary management and clinical audits.
Analyse medicines data and identify risks and areas for improvement.
Provide education and training to pharmacy and healthcare colleagues.
Undertake clinical screening and final accuracy checks when working in the dispensary.
