Job summary

Oxleas NHS Foundation Trust is seeking a Band 7 Prison Services Clinical Pharmacist to lead medicines optimisation within HMP Guys Marsh and our integrated healthcare team. You will deliver structured reviews, manage repeat prescribing and provide medicines advice in a secure environment.

This role requires strong clinical knowledge, leadership, and collaboration with GPs, nurses, and multidisciplinary teams. An enhanced DBS check and prison vetting are required; sponsorship is not offered.

Qualifications

• Master’s degree in Pharmacy or equivalent and GPhC registration.
• Diploma in Clinical Pharmacy or equivalent, or currently working towards.
• At least three years’ experience delivering clinical pharmacy services in relevant settings.

Are you an experienced clinical pharmacist looking for a rewarding role where your expertise can make a real difference? We are seeking a motivated Band 7 Prison Services Clinical Pharmacist to join our integrated healthcare team at HMP Guys Marsh.

You will lead medicines optimisation for patients with long-term and acute conditions, manage repeat prescribing, and deliver structured medication reviews and minor ailment clinics. Working closely with GPs, nurses, pharmacy colleagues, mental health and substance misuse teams, you will support the safe, effective and patient-centred use of medicines within a secure environment.

Detailed job description and main responsibilities

You will work autonomously and as part of a multidisciplinary team to identify patients at risk of medicine-related harm, optimise treatment and reduce avoidable hospital admissions. You will develop and maintain repeat prescribing procedures and searches within SystmOne, support long-term condition clinics and provide specialist pharmaceutical advice to healthcare professionals and patients.

The role includes contributing to clinical governance and medicines management committees, developing policies and procedures, completing audits, supporting public health initiatives and implementing improvements arising from national guidance and medicines safety alerts.

You must hold a Master’s degree in Pharmacy or equivalent and be registered with the General Pharmaceutical Council. You will have relevant clinical pharmacy experience, strong clinical reasoning and communication skills, and a commitment to continuous professional development. Experience within prison, GP, hospital, community, mental health or substance misuse services would be advantageous.

The successful candidate will be required to complete an enhanced DBS check and prison security vetting.

Qualifications and Professional Registration

• Master’s degree in Pharmacy or equivalent.
• Current registration with the General Pharmaceutical Council.
• Diploma in Clinical Pharmacy, General Pharmacy Practice or equivalent, or currently working towards this.
• Evidence of continuing professional development.

Relevant Clinical Pharmacy Experience

• At least three years’ experience delivering clinical pharmacy services.
• Experience in a UK prison, hospital or community pharmacy setting.
• Experience undertaking medication histories, structured medication reviews and medicines optimisation.
• Experience working within multidisciplinary teams and providing prescribing advice.

Clinical Knowledge and Skills

• Recent clinical practice in a GP, hospital, or prison setting.
• Strong clinical reasoning and ability to interpret evidence and make safe recommendations.
• IT literacy; SystmOne experience is desirable
