The Forward Trust is seeking a dedicated Recovery Practitioner to support prisoners aged 18 and over within an integrated healthcare framework. The role focuses on psychosocial drug and alcohol interventions and accredited treatment programmes using a person-centred approach and harm reduction strategies.

Key duties include SMART assessments, one-to-one sessions, group facilitation, and collaboration with healthcare, probation and other partners to ensure coordinated care and successful recovery

Qualifications

• Experience in mental health or substance misuse services.
• Experience carrying out comprehensive assessments and SMART care plans.
• Experience delivering structured one-to-one interventions.
• Facilitating therapeutic groups.
• Using motivational interviewing techniques in clinical settings.
• Applying CBT and motivational counselling skills.
• Strong record-keeping and use of relevant systems.

Responsabilités

• Completing comprehensive assessments and developing SMART care plans tailored to individual needs.
• Providing harm reduction advice, information and guidance.
• Delivering structured one-to-one interventions and therapeutic group sessions.
• Delivering structured treatment programmes where relevant to the service.
• Using evidence-based approaches, including motivational interviewing, CBT and motivational counselling techniques.
• Supporting clients to engage with their recovery and make sustainable changes.
• Collaborating with healthcare, probation and other partner agencies to ensure coordinated care.
• Supporting throughcare and resettlement with housing, employment and external drug intervention teams.
• Contributing to service targets and outcome measures set by Forward and local commissioners.
