Job summary

The Forward Trust in Chelmsford is seeking a Drug and Alcohol Practitioner to support prisoners with substance misuse, delivering client-centred care using harm reduction and abstinence-based approaches. You will work within an integrated healthcare team to help clients reduce harm and prepare for community re-entry.

Key duties include caseload management, assessments, care planning, harm reduction guidance, and facilitating 1:1 and group sessions, with a focus on transition and throughcare

Qualifications

Experience in substance misuse or related support needs.
Able to complete effective assessments and develop care plans.
Good written communication and report-writing skills.

Responsibilities

Manage a caseload of clients with problematic substance misuse.
Provide client-centred, evidence-based support using harm reduction and abstinence-based approaches.
Complete comprehensive assessments and care plans based on individual needs.
Provide harm reduction advice, information and guidance.
Deliver 1:1 sessions and group-work on substance misuse and low-level mental health.
Support transitions and resettlement planning for release.
Coordinate with internal and external services to ensure appropriate support.
