Job summary

Forward Trust in Swansea is seeking a Recovery Practitioner to support prisoners aged 18 and over within an integrated healthcare framework. You will manage your own caseload and provide high‑quality support throughout the treatment journey.

The role includes assessments, SMART care plans, harm reduction guidance, individual and group sessions, and evidence-based approaches such as motivational interviewing and CBT, working with partner agencies to ensure coordinated care and effective

Qualifications

Compassionate and resilient approach to supporting people.
Experience in mental health or substance misuse services preferred.
Experience with SMART care plans and assessments.
Strong IT skills and accurate record keeping.

Responsibilities

Completing comprehensive assessments and developing SMART care plans.
Providing harm reduction advice, information and guidance.
Delivering structured one-to-one interventions and therapeutic group sessions.
Delivering structured treatment programmes where relevant to the service.
Using motivational interviewing, CBT and motivational counselling techniques.
Supporting clients to engage with their recovery and make sustainable changes.
Working with healthcare, probation and other partner agencies for coordinated care.
Contributing to throughcare and resettlement activities and service targets.
