Job summary

The Forward Trust is seeking a Health and Wellbeing Practitioner to join our team at HMP Bronzefield in Sutton. The role provides recovery-orientated health and wellbeing services to adults with mental health or substance misuse needs within a multidisciplinary team.

You will carry a caseload, deliver one-to-one and group interventions, and develop SMART care plans with a trauma-informed, person-centred approach. Weekend rota work is required.

Qualifications

• Experience in mental health or substance misuse services with structured interventions.
• Ability to carry out comprehensive assessments and design SMART care plans.
• Experience facilitating group-work sessions and delivering individual interventions.
• Proficient in maintaining accurate electronic records and data reporting.

Responsibilities

• Manage a caseload and provide person-centred support to adults 18+ in a prison setting.
• Deliver harm reduction guidance and complete comprehensive assessments.
• Develop, implement and review SMART care plans with service users.
• Deliver one-to-one and group interventions and use motivational interviewing.
• Support release planning and through-care with community partners.
• Maintain timely records and contribute to service monitoring and outcomes.
• Work on a rota to cover weekends; comply with vetting and safeguarding.
