Job summary

The Forward Trust is seeking a Health and Wellbeing Practitioner for HMP Brixton to deliver drug and alcohol recovery support to men in custody. The role is full-time (35 hours per week) with a salary from £27,000 depending on experience and experience-based progression.

You will manage caseloads, run individual and group interventions, and work with healthcare colleagues to support resettlement and wellbeing. DBS checks apply.

• Good organisational skills with ability to manage a caseload and maintain accurate records.

Responsibilities

• Manage a caseload, completing assessments and developing individual care and recovery plans.
• Deliver one-to-one interventions, group sessions and structured treatment programmes.
• Provide harm reduction advice and support to people experiencing drug and alcohol misuse.
• Coordinate care with healthcare professionals and partner agencies to support resettlement.
• Monitor progress, maintain records and contribute to service targets and quality standards.
• Identify risks and safeguarding concerns to ensure the safety and wellbeing of service users.
