Forward Trust is seeking a Senior Health & Wellbeing Practitioner to lead the Integrated Substance Misuse Team at HMP Usk & HMP Prescoed, combining frontline clinical work with team management and service development.

You will support adult males with substance misuse and mental wellbeing needs, delivering 1:1 and group interventions, conducting assessments, and working with multi-agency partners to achieve positive outcomes.

Qualifications

• Experience in substance misuse, mental health or wellbeing services.
• Strong leadership and supervisory experience.
• Confidence delivering both group and individual interventions.
• Ability to build effective working relationships across agencies.
• Organised, proactive and solution-focused approach.
• Commitment to high-quality, person-centred support.
• Welsh and English speaking preferred.

Responsabilités

• Line managing Health & Wellbeing Practitioners and supporting development.
• Deliver 1:1 interventions and group work across caseloads.
• Carry out assessments, care planning and referrals.
• Work with healthcare teams and multi-agency partners.
• Support service performance, outcomes and reporting.
